Notice of Privacy and Terms of Service
University General Dentists is required by law to maintain the privacy of those we provide services to. By scheduling an appointment, receiving services, using our website, or opting into communication systems, the patient agrees to our terms of service.
Financial Obligations
Payment is due at the time of service. Estimates of cost for some services may be provided, but it is expected that patient payments are made at the time of service. We reserve the right to reschedule or cancel an appointment if payment is not rendered.
As a courtesy, our office will files insurance claims but we are not responsible for insurance company payment decisions. This includes filing with in-network and out-of-network providers.
Late payments may be subject to late fees. Past due balances may be sent to collection agencies after an extended period of time.
Appointments and Cancellation Policy
At UGD, we ask that patients please cancel an appointment at least 24 hours in advance. Continued missed appointments may result in dismissal as a patient.
Use of Electronic Communications (SMS and Email)
Communication in the form of electronic communications, such as SMS, email, and patient portals, is for non-emergency matters only and may involve security risks even with reasonable safeguards. For medical emergencies, patients should call 911 or an emergency service provider.
Obtaining Communication Consent
All patients must provide consent to be contacted electronically. Information provided will not be shared by a third party and will be used for patient communication practices only. These practices may include but are not limited to appointment reminders, office closure updates, in office information, etc.
SMS Messaging
Patients who opt-in to SMS as part of the consent process will not have their information shared with third parties. Communication is limited to the reasons outlined in “Obtaining Communication Consent”. Messaging frequency may vary. Message and data rates may apply. You can opt out at any time by texting STOP.
Email Messaging
Patients who opt-in to email as part of the consent process will not have their information shared with third parties. Communication is limited to the reasons outlined in “Obtaining Communication Consent”. Messaging frequency may vary. You can unsubscribe at any time by using the “Unsubscribe” link at the bottom of the email.
